• Application form for Schengen Visa,
completed, signed and dated.
• Appendix D – Family Appendix, completed
• Passport or other travel document (Please read the notifications below.)
- Your passport must be issued within the last ten (10) years
- Your passport must have two (2) blank visa pages
- Your passport must be valid at least three (3) months afther your visa expires.
• Copies of the first 10 pages, all pages with stamp and the last page of Passport or other travel document
• One (1) photograph 35 x 45 mm in colour not older than one year. The photograph
must show the full face and be taken against a plain, evenly lit and light background.
• Extract from the Turkish family book (Nüfüs) showing parents, siblings etc. The
document should not be older than three (3) months.
• Invitation letter from the organization in Sweden.The invitation must contain
details such as:
Personal particulars of the applicant
The purpose of the visit
Arrival and departure dates
Who will cover the travel and hotel expenses
• Letter in English (in original) from the organization in Turkey.
The letter must contain details such as:

Arrival and departure dates
Who will cover the travel and hotel expenses
• Written parental consent, in original, or minors (age under 18).
This document should be stamped by a Notary Public in Turkey.
• A proof of valid personal medical travel insurance.
Additional documents may be required after the examination of the application by
the Consulate General of Sweden.
